When to use this tool

Use the check sheet, or tally sheet, to collect data (e.g. the number of defects in a particular process or by a particular operator) on a regular basis over a period of time.

How to use this tool in EngineRoom

  1. Open the Check Sheet template from Measure in the DMAIC menu or Quality Tools in the Standard menu.
  2. Click Launch to open the template in a new tab, and name the study.
  3. Enter the name of your first metric at the top of the first section.
  4. In the column at the left, enter the first time period corresponding to the data being collected.
  5. In the main section of the template, enter the number of trials or sample size inspected along with the number of defects or rework in that sample. Once these two numbers are entered in the table, the Rate column automatically calculates the percent defects or rework during that time period.
  6. Continue entering the data in a systematic manner for other metrics and time periods until you have enough data to cover a few time periods, clicking the ‘plus’ buttons to add rows or columns as needed.

A good check-sheet should show:

  • what is being measured.
  • who is collecting the data.
  • the time period associated with the data.
  • a clearly marked space to record the data.
  • stratification of the data into useful categories

Note: As the sections of the form are filled out, the Totals table on the right of the template will populate with the total trials, counts of defects or reworks and the corresponding defect or rework rate across all the categories of metrics in the table.
